Name Change

• Complete your passport application using the government online application here.
• Print, sign and send us two (2) copies of the completed passport application. Only the final 2 pages of the application, excluding the instructional page, are required.
• At this time, expediters are not allowed to expedite applications with the gender marker X selected.

For a name change, it is necessary to send in your original and most recent passport, which meets following requirements:

• It is in good condition (no damage).
• It was issued when you were 16 or older: it is a 10-year validity passport.
• It was issued less than 15 years ago.
• If you are updating both your passport book and card, the same conditions mentioned earlier apply, and you must submit both the original documents.

Please provide one (1) recent passport photograph, captured in the last six months, printed on photo paper.
For your convenience, you'll have the option to upload a digital photo* (additional fee applies).
*Face the camera directly without objects behind you and do not wear eyeglasses, headphones, or earbuds (click for sample).

Include a check or money order payable to the U.S. Deparment of State in the amount of:

• $211.36 for passport book request, or
• $241.36 for passport book and card request.

Important: Starter/temporary checks are not accepted. You typically receive this when you just opened a bank account.
For your convenience, you'll have the option to add the government fee to your order, charge it to your credit card, and we will take care of it.

To obtain your new passport with your updated name, you must provide:

• Original or certified copy of your marriage certificate.
• Original or certified copy of court document indicating your new legal name under federal or state law.
